RESIDENTS
RIGHTS
1. Every Resident has the right to be treated with respect, compassion and dignity.
• To have my choices respected and supported when doing so does not interfere with the rights of other residents
• To have my concerns or those of others heard and addressed
• To communicate in confidence and present concerns without fear
• To have my decision making, capabilities and independence and respected
2. Every Resident has the right to exercise rights as an individual, as a citizen and as a resident.
• To have the opportunity for my emotional, physical, social, cultural and spiritual needs to be met
• To have a designated substitute decision maker exercise rights on my behalf when needed
• To participate with other residents in a Resident Council
• To vote for any candidate of my choice in public elections
3. Every Resident has the right to be informed.
• To know who is responsible for my direct care
• To be informed about my medical condition and any treatments in words I understand
• To be aware of the consequences of giving or refusing consent for treatment
• To be informed when service charge increase
4. Every Resident has the right to safe quality care and to be protected.
• To be provided exceptional, individualized quality care in a clean, safe environment
• To be protected from any abuse, neglect, or discrimination and to feel safe and secure
• To participate in the development of my care plan
• To participate in my interdisciplinary care conference with my family, friends or representative of my choice
• To be free from any restraint except when my safety or the safety of others is at risk
5. Every Resident has the right to privacy
• To have privacy when being examined or receiving care
• To receive advance notice when others come into my personal space
• To have all care related information held in confidence
6. Every Resident has the right to receive visitors and to communicate in private
• To invite families and friends to visit with me in my home
• To have the freedom to choose who visits me
• To conduct private personal business
• To be provided with a private setting for visits when necessary
7. Every Resident has the right to manage their finances when able to do so
• To have funds retained in a Guest Trust Account
• To access my funds for the purchase of items desired for my personal comfort or interest
• To designate a person to purchase items on my behalf
8. Every Resident has the right to maintain personal possessions and space
• To have a designated area that is considered my own space
• To retain a reasonable amount of personal items as space permits unless doing so infringes on the safety, rights or health of others
• To relax in shared areas designated for my comfort, leisure and enjoyment
Responsibilities
• To treat all with dignity, consideration and respect in a manner that fosters living and interacting in harmony with other residents and the care team
• To be well informed regarding our Respectful Home guidelines and communicate any concerns in a timely and courteous manner
• To provide correct and complete information and stay informed on how your personal choices affect your health
• To adhere to Glen Haven Manor’s policies and safety standards
• To be accountable and care for my personal valuables and possessions
• To honour all fees, payments and expenses as they become due
FAMILIES
RIGHTS
1. To be treated with dignity, respect and kindness
2. To be accepted as a valuable and active partner in care for my loved one
3. To be informed about my loved one through the designated family contact and to have any concerns addressed in a timely manner
4. To visit with loved ones in accordance to the policies and safety standards of Glen Haven Manor
5. To participate with other families at the Family Council assemblies
6. To be able to communicate frequently with my loved one through phone, Facetime, or Skype
RESPONSIBILITIES
1. To respect the rights of all residents, treat them with dignity, compassion and kindness and follow confidentiality policies
2. To respect the roles and responsibilities of the management and staff of Glen Haven Manor in a manner that fosters a collaborative working relationship, free from harassment
3. To be well informed to Glen Haven Manor’s Respectful Home guidelines as well as notices and information placed on Glen Haven Manor’s web site and/or social media platforms
4. To identify substitute decision maker or family contact
5. To communicate any concerns in a punctual and courteous manner through the designated contact
6. Participate in interdisciplinary care conferences as a substitute decision maker or as requested by your loved one or Glen Haven Manor
